This comment is definitely late, but it's been a while since I've come by your blog (longtime reader who has never before had the nerve to comment!) and when I stopped by today was excited to see this post as I just started learning about STEMI in medic school this very morning and got to feel excited when I recognized information here and thrilled to pick up new things my teachers didn't mention.

Thank you for the supplemental info and inspiration regarding how to handle these calls!

Thanks again for the comments. It is very interesting hearing how other systems are handling STEMIs. Overall, the trend is clear and moving in the right direction.

As far as an editor, I am a crazy rewiter, but sometimes rewriting, I end up missing a word or changing a tense. I'm not certain why I had so many typos in my inital draft. I think maybe not being familiar with the new blog site and its intricacies, I may have not saved changes I thought I'd made.

Any rate, I have a new STEMI post that should be up tomorrow, and hopefully it will be typo free.

Thanks again,

Best I've seen is 11 minutes door to (walking towards the) lab. And that was with a transmitted 12-lead from the field. Tends to happen when you give the triage nurse control over whether or not Cardiology is notified pre-arrival.

Worst.... well, let's just say I was bringing in my next patient before they started moving. And that was after the 911 call, response, onscene time, transport to the community hospital, workup, call for transfer, intubation, transport time......... Yeah. It wasn't pretty from the getgo.

Peter, maybe you need an editor for your blog! ;)

Nice post. STEMI calls are indeed good EMS calls. Well, good for us, not so much for the patient.

Anyway, our protocol is similar to yours except we don't do Johnnies, only do one IV, and we don't send EKGs. We do have a cath lab hospital only point of entry plan.

The best door to balloon time I have seen is fifteen minutes. The best door to cath lab time is two minutes. Same hospital. The worst door to cath lab time I've seen is 12 minutes. Well, that was door to starting to move to the cath lab which is in a different building on the campus. Not so good.

I've heard a rumor that we are going to start bypassing EDs and going directly to the cath lab. It's been a long journey, but we should soon be there. I hope.
